Medicaid Solutions for Elderly and Disabled Individuals

The Elderly and Disabled Waiver Program is a Medicaid home and community-based waiver services program that provides community-based social, health, and support services to eligible consumers as an alternative to placement in a nursing home.

We also offer the following:

  • Independent Care Waiver Program (ICWP)This program offers services that help a limited number of adult Medicaid members with physical disabilities live in their homes or the community instead of a hospital or nursing home. ICWP services are also available for persons with traumatic brain injuries (TBI) Traumatic brain injury occurs when the head receives a blow that is enough to shake the brain and damage it. Elders commonly sustain this type of injury due to falling. While recovering from this type of injury, it can be dangerous to leave the patient alone. For this reason, we are here to provide our services. As a member of the Brain Injury Association (www.biami.org), we will make sure that the patient is safe and fully comfortable while they are recovering from their injury. Must be between the ages of 21 and 64 when they apply for service and do not have a primary diagnosis of a mental disorder (intellectual disability/mental illness)
  • Community Care Service Program (CCSP) The Community Care Services Program (CCSP) is a Medicaid home and community based waiver services program that provides community-based social, health and support services to eligible consumers as an alternative to placement in a nursing home.
  • Service Options Using Resources in a Community (Source) Services Options Using Resources in Community Environments (SOURCE) Program provides long-term home and community based services (HCBS) for state residents who are aged and disabled. Intended for persons who are functionally impaired and require assistance with Activities of Daily Living, such as bathing, dressing, mobility, and eating, a Nursing Home Level of Care is required.
  • Structure Family Care Services(Must currently be enrolled in Source and CCSP) This program provides support to family caregivers who live with and assist elderly and or disabled waiver participants requiring significant assistance with activities of daily living
  • Money Follows the Person (MFP) The Money Follows the Person (MFP) demonstration supports state strategies to rebalance their long-term services and supports systems from institutional to community-based care. MFP has played a key role in many states’ long-term services and supports (LTSS) rebalancing efforts under the Medicaid program providing flexible funding opportunities to help states develop and test the necessary processes, tools, and infrastructure to advance LTSS system reform and to support successful transitions from institutional to community-based settings for individuals eligible for Medicaid LTSS.
